The Program

One Door Mental Health has been contracted by Central Eastern Primary Health Network to provide the Primary Integrated Care Support (PICS) Program.

The aim of PICS is to provide evidence based clinical mental health services and links to health and psychosocial supports in a primary health care setting, to meet the needs of people who experience severe mental illness.

PICS is a person-centred service, and this provides the flexibility to support people to access the supports and services they need, so when they are discharged to care of their GP, they stay well living in the community.

About the Role

The Peer Worker will work as a team member along with PICS Credentialed Mental Health Nurses to provide support to meet each person’s needs and wishes to enable them to live well and achieve their recovery goals. Key responsibilities will include:

  • Build one to one supportive relationships based on Peer Support Values and Principles that focus on the use of lived experience to support the people we work with in regards to the persons needs and recovery
  • Work with the nurses, in the development of the individual care plans with clarity of identified roles and tasks.
  • Provide ongoing support to the client within the context of the care plan and goal setting.
  • Provide the client with education and strategies for the development of decision-making, self-advocacy, self-management and wellness skills using personal lived experience.
  • Assist the client to navigate the system including building knowledge of current services in the area and referring to other services.

About You

To succeed in this role, you will have:

  • Personal Lived Experience of living with a Mental Health condition, hardship and Recovery
  • Certificate IV in MHPSW (CHC43515) or willingness to obtain within the first 12 months of employment
  • Experience working in the community
  • Drivers license and access to a comprehensively insured motor vehicle
