We are seeking applications for a Support Coordinator / Recovery Coach to join our team. Hours and days can be flexible and negotiable to promote a work / life balance.
Your responsibilities will include delivering tasks in the areas of:
- Provide a person centred approach to promote Choice and Control.
- Meet with participants and carry out assessments, action planning, support planning, risk and safety planning to support disability related needs.
- Capacity and skill building activities using a basic understanding and ability to interpret NDIS legislation and funding including the NDIS price guide & the NDIS Outcomes Framework.
- Planning sessions to support participants to understand supports in their plan and utilise networks and resources to identify, link and coordinate supports for participants including community, informal and funded supports.
- Care Team meetings, report writing, letters of support.
- Collaboration between all interfaces of education, health, justice, housing and disability where needed.
- Liaise with and make referrals to appropriate services which may include: Allied Health, Community Supports, Assistance with Daily Living, Assistive Technology.
- Empower participants to activate their NDIS plans and ensuring outcomes are met through their allocated support coordination / recovery coach hours.
- Use programmes to build budgets across duration of plans.
WHAT YOU WILL NEED TO SUCCEED:
- Completion of relevant study eg: Diploma of Community Services (DCS) / Diploma of Mental Health (DMH).
- An understanding of the NDIS and disability sector.
- A passion for supporting people with disabilities to achieve the best they can – bring your special talents to their experience!
- Excellent communication skills: ability to prevent and resolve issues and to build and maintain positive relationships with all stakeholders.
- Ability to promote community inclusion and person-centred support that exceeds standards and is delivered within the available resources – be creative!
- Effective written communication skills and computer literacy to complete appropriate documentation including but not limited to: wellness plans, action plans, case notes and report writing.
- Resourceful and innovative approach to creating opportunities for those we support – make social change happen.
- Experience in delivering on individualised plans.
- Excellent time management skills and the ability to think laterally.
- Ability to work as part of a team and independently.
- Commitment to exceptional service delivery and a demonstrated professional approach to work.
- Commitment to the principles of WHS, EEO, Cultural Diversity and all relevant legislation.
HIGHLY REGARDED SKILLS, EXPERIENCE (INCLUDING LIVED EXPERIENCE), QUALIFICATIONS IN:
- Forensic, criminal justice, prison, child protection.
- Hoarding and squalor.
- Mental Health including crisis, trauma, dual diagnosis.
- Alcohol and Other Drugs.
