About the role

In this role you will work closely with the Team Leader and Community Support Workers and bring specific experience and knowledge of the Mental Health and Alcohol & Other Drugs referral pathways.

Our Homelessness teamwork as a team to provide a safe and supportive environment for service users, volunteers and staff. They actively engage in risk assessments and provide interim / short term case management and referrals specific to the individual need of the client.

They actively engage in risk assessments and provide interim / short term case management, case coordination and referrals specific to the individual need of the client and group programs.

You will:

  • Provide high quality support to participants both as individuals and in small group settings
  • Assess client’s wellbeing, mental health, physical health, recovery and opportunities
  • Develop service networks and specialist pathways for mental health and AOD referrals and supports
  • Support clients to maintain and connect with family, friends, peers, community, and other services and supports
  • Maintain accurate and up to date documentation in the Specialist Housing Information Platform.

Key requirements

  • A values-based approach to work aligned with our mission
  • Previous experience in a direct service delivery role within mental health, homelessness and/or the wider community sector, with experience working in the sector
  • Skills and experience to develop support plans (and strategies) which assist participants to achieve goals that support participants with recovery harm minimisation
  • Experience working with people with complex needs and engaging vulnerable clients whilst maintaining clear boundaries and accountability
  • Excellent communication (verbal and written) skills
  • Clear and Current National Police, Working with Children and current valid Driver's license
  • Strong Microsoft Office skills and capacity to use case management software Specialist Housing Information Platform
  • A strong service orientation and a willingness to pitch in and help others

Relevant tertiary qualifications (Certificate IV or above).
