We're an organisation providing independent Support Coordination to people living with disability under the NDIS. We pride ourselves on providing exceptional and value-driven Support Coordination services across the Gold Coast, Brisbane and surrounding areas.
About the role
As an NDIS Support Coordinator you will explore clients individual needs and develop a care plan to link the client in with NDIS funded supports and mainstream services to overcome barriers, develop capacity and achieve goals.
Key duties include:
- Build strong relationships with clients, their families and service providers.
- Take a holistic approach to help clients address their barriers to achieve goals.
- Build clients capacity to understand their funded supports, utilise their NDIS plan and plan/track budget expenditure.
- Support clients to negotiate service agreements.
- Build clients capacity to understand their NDIS funded supports and mainstream providers available in their local area.
- Coach and support clients through scheduled and unscheduled plan review processes with NDIS.
About you
- A minimum of 1 year experience working as a Support Coordinator or in a similar role.
- A minimum of Diploma in Community Services or relevant experience.
- Valid drivers’ licence, First Aid Certificate, Police Check, Blue Card and NDIS Worker Screening Check
- Experience handling complex situations and ability to resolve issues.
- Strong communication and interpersonal skills.
- Ability to work under pressure and multitask.
- Attention to detail and organised.
- Self-motivated and able to work autonomously.
- Friendly, approachable and able to work in a team environment.
