About the Role

The Client Pathway Assessor will be responsible for conducting a comprehensive assessment for any of SSI’s client who has completed the screening phase of the client pathway. As the team provides a centralized screening and intake service for the entire organization, the Assessor must possess a broad understanding of the various programs offered. This knowledge will enable them to build and maintain relationships across the organization while ensuring compliance with all contractual obligations under the relevant funding programs.

The Client Pathway Assessor will utilise SSI’s life domains—such as Health & Wellbeing, Economic Participation, Safety & Stability, Learning & Growth, and Social Inclusion—to identify each client’s unique needs and vulnerabilities. They will collaborate with clients through a client empowerment approach to establish meaningful and attainable high-level goals that align with ICMS requirements and SSI’s client-first model.

The role will provide support to clients in the interim whiles their assessments are being processed – this will include providing information, advice and referrals as needed.

Additionally, the Client Pathway Assessor role will support the continuous improvement of clients’ pathway to ensure best practices for a client-centric and efficient delivery of Service across SSI.

Essential Criteria

  • Tertiary qualifications in Community and Human Services, Social Work or relevant Discipline
  • Demonstrated experience in the delivering culturally responsive case work services
  • An understanding of skills and techniques to build clients capacity to become independent in Australia
  • Comprehensive knowledge and understanding of the multicultural sector including relevant government policies and programs
  • Advanced interpersonal and communication skills including the ability to deal constructively with complex and sometimes conflicting situations
  • Highly developed written and computer skills across a range of applications including client records management systems.

Desirable Experience

  • Minimum 3 years experience in community sector knowledge in Settlement, Employment, Health and Disability service systems.
  • Experience working with CALD communities, DFSV, Mental Health, Child safety and child engagement practices.
  • Experience working with contemporary social work and assessment frameworks including conversational style assessment of needs and goal setting.
  • Lived experience of forceful displacement and migration
