In this rewarding role you will be providing care coordination, intensive case management and specialist support for clients who are seeking assistance with alcohol and other drug (AOD) use and other complex support needs.
You will undertake person-centred comprehensive assessments and care planning in conjunction with the client and use your knowledge and skills of the AOD and other sectors to effectively provide tailored care, that assists individuals achieve their care plan goals. In this diverse role you will be delivering services on an outreach basis, meeting with clients in various locations, onsite and virtually.
