Support Chain Pty Ltd is a small, family owned, registered National Disability Insurance Scheme (NDIS) provider servicing the southeast suburbs of Melbourne through to the Mornington Peninsula. We provide Support Coordination, Specialised Support Coordination, Plan Management and Support Workers to NDIS participants.  We have seen significant growth since our inception 3 years ago and are known as one of the most respected companies providing services by the NDIA and its community partners.

An exciting opportunity for an enthusiastic, organised and self-motivated person is now available in the casual full-time role of Support Coordinator.  Our Support Coordinators work from home as well as meet regularly for face-to-face Staff meetings and visit participants in their home so you will live in the south eastern suburbs or surrounds, including Bayside and Mornington Peninsula regions of Melbourne.

The Position – Support Coordinator

In this role, you will:

  • Work with NDIS participants, their carers and families to build their capacity to understand their NDIS Plan and help implement services and supports according to their goals and funding
  • Research and liaise with service providers to ensure exceptional care is delivered to participants
  • Build respectful and strong relationships with disability providers, mainstream services, NDIA and community partners
  • Enter accurate case notes in a timely manner in our CRM system Careview
  • Organise and manage a caseload of participants
  • Schedule and attend Care Team meetings as necessary
  • Actively participate in Staff and Team meetings
  • Complete reports within the required timeframe according to NDIA reporting requirements
  • Operate within relevant legislation, NDIA policies and guidelines
  • Actively participate in company initiatives to maintain, build upon and promote a positive and collaborative workplace
  • Undertake continuing professional development to stay up-to-date with the ever changing Scheme

Candidate - You

To be considered for this position, candidates must have the following experience and skills:

  • Understand the role of a Support Coordinator within the NDIA
  • Experience or qualification in the disability, housing, health or welfare sector (desirable)
  • Understand the NDIS, NDIS Code of Conduct and its guidelines and rules
  • Ability to follow our company’s policies and procedures and ensure compliance with relevant statutory requirements
  • Excellent written and oral communication skills
  • A high level of organisational skills with the ability to meet deadlines, prioritise demands and manage time effectively and efficiently
  • Ability to work autonomously and within a team environment
  • Sound knowledge of Microsoft Word, Outlook, Teams and Sharepoint
  • Ability to learn quickly, adapt and problem solve
  • A great can-do attitude, with a friendly and warm demeanour with participants and their supports

Other Requirements

To be considered for this role you must have (or be prepared to obtain) the following:

  • A current Working with Children Check
  • A clear NDIS Worker Screening Check
  • Proof of Identity
  • Current First Aid and CPR
  • Proof of Right to Work in Australia
