VincentCare Victoria is looking for an Intensive Case Manager to join our Journey to Social Inclusion (J2SI) program. J2SI is an innovative program focused on breaking the cycle of homelessness through intensive, person-centered support. This role offers the opportunity to provide long-term, trauma-informed case management and service coordination, supporting individuals to achieve stable housing, independence, and social inclusion.

What You’ll Be Doing:

• Providing intensive case management with a small caseload, supporting clients to secure and maintain stable housing.
• Using a trauma-informed approach to help clients develop life skills, improve well-being, and foster social inclusion.
• Working in an outreach capacity, meeting clients in the community.
• Collaborating with ACCOs, local service providers, and key stakeholders to ensure holistic support.
• Using persistent and assertive engagement techniques to maintain professional relationships with clients.
• Monitoring client progress and maintaining accurate records in line with program requirements.

What We’re Looking For:

• Minimum 3-year degree in social work, psychology, or a related field (or equivalent experience).
• Experience working with homelessness, mental health, AOD, or family violence support services.
• Strong understanding of case management, risk assessment, and trauma-informed practice.
• Excellent communication skills and ability to engage with diverse stakeholders.
• Cultural awareness and ability to provide culturally safe support, particularly for First Nations clients.
