I’m Blake: a young man with a cheeky sense of humour. I spend time volunteering, exploring the world on walks or drives in the car, and chilling-out at home. I am a Christian with strong links to a local church.
I happen to have an intellectual impairment and am non-verbal. I need support in day-to-day living, in my interaction with others, and in participating as a valued member of wider community groups.
My support team is working to empower me to reach my goal of living independently like other men in their twenties. I am looking for a therapy coordinator to join my team. This person will help my team to build upon my achievements and develop new ones.
“Therapy Coordinator”
4 hours per week, most shifts Monday-Friday
- Review, plan and set my allied health goals (such as speech and language and occupational therapy)
- Support me to achieve my therapy goals by coordinating and attending therapy sessions with/for me, in-person or online
- Convey my allied health and therapy goals to my team of support workers and to family members so they can use therapy strategies, activities or exercises in my day-to-day life
- Observe support workers as they implement the home therapy program and give feedback to them
- Create training materials where appropriate
- Document my progress and milestones achieved in the home setting and report these back to therapists and to my parents
- Maintain regular contact with all parties to ensure that the home program is running smoothly
- Problem solve issues as they arise
- Attend team meetings (once per month – in person; once per month – online)

Criteria
- Australian Driver’s Licence (essential)
- Experience working alongside allied health workers (desirable but not essential)
- NDIS Workers Clearance
