The Cornelia Program is a collaboration between the Royal Women’s, HousingFirst and Launch housing - a new multi-agency service focused on providing support and housing alongside healthcare to mothers and babies experiencing homelessness.
As the Cornelia Care Coordinator you will play a vital role in assessing and screening women being referred into the program from within the Women’s and externally. You will also support a caseload of women and babies in the program through individual case management. You’ll also be involved in complex assessments and referrals to specialist community-based services such as Alcohol & Other Drugs (AOD) and child protection.

You will be an experienced social worker or other health practitioner, who is able to work well in a small multi-disciplinary team across the hospital and community sites.
