  • $102,091 - $116,646 (pro-rata for part time) + salary packaging + super + 5 weeks annual leave

About the Program

The Cornelia Program is a collaboration between the Royal Women’s, HousingFirst and Launch housing - a new multi-agency service focused on providing support and housing alongside healthcare to mothers and babies experiencing homelessness.

About the role

As the Cornelia Care Coordinator you will play a vital role in assessing and screening women being referred into the program from within the Women’s and externally. You will also support a caseload of women and babies in the program through individual case management. You’ll also be involved in complex assessments and referrals to specialist community-based services such as Alcohol & Other Drugs (AOD) and child protection.

A regular day will see you:

  • Provide complex assessments and risk assessments of women being referred into the Cornelia program
  • Complete necessary documentation for referrals to HousingFirst
  • Liaise with partner organisations, internal hospital programs and external stakeholders
  • Make and receive referrals from relevant community and health services

About you

You will be an experienced social worker or other health practitioner, who is able to work well in a small multi-disciplinary team across the hospital and community sites.

You’ll bring with you:

  • Relevant tertiary qualifications and registration and/or membership (i.e. Social Work, Midwifery etc)
  • Excellent assessment skills
  • Experience working with women with complex needs in AOD, Mental Health and Family violence
  • Experience of working in multidisciplinary teams
  • Good organisational and communication skills
