Are you interested in making a real difference? Are you looking for a dynamic valued role, then come and join Tim’s Team supporting a delightful man in his 30s, who needs 24 hour support in the lower Blue Mountains area.
With Tim’s changing health needs as he is ageing, Tim needs support from people who are observant, creative thinkers and collaborative problem solvers to join his great team.
If you have health or dementia related skills and can bring fun and energy, while managing complex care, then Tim needs you!
Tim requires 24 hour active support across various shifts to enable him to live his good life. We negotiate rostering flexibly and match team member’s strengths and interests to enrich Tim’s life. Rostering is generally negotiated 6 monthly allowing both Tim and his team to plan ahead.
Tim needs support with all areas of his life and is looking for team members who will collaborate to facilitate, support and guide Tim to be as independent as possible.
Tim has a great group of friends and valued roles in the community and needs support to maintain these. This includes everything from FaceTiming with cousins, hosting friends for music around the fire pit, going to the pub to play pool with a friend, staying fit, to busking and volunteering.
Tim’s health needs are becoming more complex so team members will need to be observant and alert, to continue to learn and adapt to his changing needs and facilitate Tim to continue his full valued life with fun, family, friends, fulfilment and purpose, while managing his complex care.
Team members will need to be up to the challenge of the increased complexity of health needs and committed to joining in ongoing learning and development.
Tim is a 35 year old man who loves life. Tim has Down Syndrome, early onset ageing and has increasing complex health needs. Tim lives in his own home with 24 hour support.
With changing health needs, Tim is slowing down from the fast pace of life. He is passionate about music, drumming, steam trains and loves gardening, swimming and more. Tim is a people person, with a great sense of humour and has a great social network of friends and extended family, he wants to maintain.
Tim has multiple valued roles in the community, such as volunteer at the Train Museum and Community Garden, jamming at a music studio and hosting neighbourhood garden gigs. Tim loves life!
Tim has a supportive family that self manages Tim’s NDIS funding and supports.
If this sounds like you, and you want to be part of this dynamic team, then why not apply today.
