About the Role

We are seeking a passionate and experienced Social Worker to join our dynamic team committed to providing person-centred, high-quality support to people living with disabilities under the NDIS framework. You'll be working closely with participants, their families, and other professionals to empower individuals to live their most independent and fulfilling lives.

Key Responsibilities

• Conduct assessments and develop holistic care and support plans
• Advocate for the rights and needs of clients within the NDIS framework
• Provide case management, psychosocial support, and crisis intervention
• Collaborate with a multidisciplinary team to deliver tailored support
• Maintain accurate, confidential records and documentation

What We’re Looking For

• A degree in Social Work (AASW membership highly desirable)
• Experience supporting individuals with disabilities or complex needs
• Strong understanding of the NDIS and community-based supports
• A trauma-informed, culturally sensitive, and strengths-based approach
• Exceptional communication and problem-solving skills
• Valid driver's license, NDIS Worker Screening Check, and WWCC
