Purpose of position:
The Support Coordinator/Psychosocial Recovery Coach assists clients to achieve their NDIS plan goals and strengthen their ability to coordinate and implement their supports, including funded and mainstream supports.
Responsibilities
- Facilitate positive outcomes for participants using a strengths and rights-based approach.
- Build individual capacity for clients and enhancing choice and control in the provision of high-quality support coordination.
- Ensure supports are provided to assist participants to achieve their goals.
- Assist participants to resolve points of crisis in their lives.
- Assist participants to access mainstream and community supports.
- Support participants to strengthen their informal networks.
- Complete progress notes and NDIS service billing in line with key performance indicators.

Qualifications and experience
- Minimum Certificate IV in disability or community work. Qualifications are not a deal breaker, we value your experience.
- Extensive experience in supporting people with disability to build capacity and access funded and mainstream services.
- NDIS Worker Screening check.
- NDIS Orientation Certificate or willingness to obtain.
- Knowledge of the NDIS line items, budget categories and price caps.
- Proficiency with technology.
- Demonstrated ability to communicate effectively within the team.
- Ability to work under pressure and meet deadlines.
- A strong sense of professional boundaries and integrity.
- Current driver’s licence and access to a registered roadworthy and insured vehicle (minimum third-party property).
- Evidence of right to work in Australia.
