Why This Role Matters

Foley House is a collaborative community residential service in Footscray supporting 45 men, including trans-men, who are homeless and facing trauma, severe disadvantage, mental illness, AOD, and chronic health issues. The service aims to maximise residents' independence through person-centred, strengths-based, and trauma-informed practices, focusing on improving health, well-being, and personal potential. The role involves leading a care team to provide holistic support during business hours, supporting the Community Support, Case Management, and NDIS teams in delivering services aligned with person-centred, strengths-based, and trauma-informed approaches, ensuring program guidelines and service delivery targets are met.

About The Role

We are seeking a dedicated Care Coordination Team Leader for our Foley house Program in a Full-time capacity. This position is based out of Footscray.

Salary and conditions are in accordance with the Grade profile SCHADS Award level 6.

Key Responsibilities:

  • Provide trauma-informed, person-centred care, promoting innovation and flexibility.
  • Support transdisciplinary approaches to meet residents' needs.
  • Ensure culturally inclusive services and a Rights-Based Approach.
  • Align service delivery with national and state plans.
  • Document service standards and address resident feedback.
  • Manage risks and maintain safety for residents and staff.
  • Establish client pathways and manage referrals to internal and external services.
  • Coordinate NDIS, Case Management, and Community Support staff.
  • Foster a culture of accountability, performance, and professional growth.
  • Lead recruitment and staff development efforts.
  • Ensure a safe and supportive work environment.
  • Oversee NDIS compliance, billing, and participant coordination.
  • Act as Foley House Manager when needed.
  • Build strong relationships with services, partners, and community providers.
  • Connect residents to TSA and external health and support services.
  • Align operations with the Foley House business plan and identify service gaps.
  • Analyse performance and implement improvements.
  • Represent Foley House in quality improvement initiatives.
  • Facilitate staff meetings and leadership activities.
  • Ensure compliance with TSA policies on performance, training, and injury management.
  • Maintain accurate resident records and conduct reporting and audits.
  • Ensure proper documentation for staff supervision and meetings.

About You:

  • Degree qualification in social work, psychology or related discipline
  • Leadership experience in the Community, Health & or Disability Services Sector required
  • Advanced Written and Oral Communication Skills
  • Ability to think strategically
  • Critical thinking / Problem solving - often within short timeframes
  • Use of de-escalation / crisis communication skills to reduce high conflict situations
  • Advanced organisational and administrative skills
  • Ability to communicate effectively with people experiencing homelessness
  • Ability and understanding of supporting staff exposed to distressing traumatic situations
  • Relevant experience in the supervision of staff
  • A Nationally Coordinated Criminal History Check
  • A current and valid Working with Children's Check
  • A valid Victorian Driver's license.
  • Disclosure of pre-existing health conditions that may impact on fulfilling position requirements.
  • Fully vaccinated for COVID 19
  • First Aid Certification: Level 1 and 2
