Bring your experience and passion to our Client Practitioner role supporting our customers to live independently at home and build inclusion within the community whilst maximising their choice and control over services they receive to live an ordinary life. This role will see you making a difference in the lives of vulnerable people, providing support coordination services under NDIS funding.

Your excellent people skills coupled with your ability to remain cool under pressure and identify and manage competing priorities will ensure your success in the role.

A clear understanding of the community services sector, and the ability to communicate and negotiate with service users and their carers is required, along with a strong focus on reaching set goals and targets.

The successful candidate will enjoy working in a flexible and constantly changing environment where time management and attention to detail are essential. The administration component of this role requires excellent administration skills, strong computer literacy, report writing and financial management abilities.

What's on offer:

  • An attractive base salary ranging from $39.62 per hour with competency based increments up to $46.52 per hour, plus 11.5% super
  • Minimum 28 hours per week up to a maximum of 35
  • Contract up until 30 June 2025, with the possibility of 12 month individual extensions to a maximum June 2029. Extensions will be based on funding availability.

We are seeking:

  • Diploma in Case Management or similar.
  • Experience working in aged or disability care in the community (3+ years).
  • Ability to undertake the functions or phases of case management as defined by the Case Management Society of Australia.
  • Experience in conflict resolution, communication and negotiation skills to deal with program clients and their carers.
