TwoScots recruitment is excited to partner with a growing NDIS provider based in the Western Suburbs. They are looking for an experienced Support Coordinator/Psychosocial recovery coach to join their close-knit team.

About the Role

As a support coordinator you will manage a case load of NDIS participants and be responsible for the following:

Duties

  • Helping clients understand their plan and use their supports flexibly and assisting them in exercising their choice and control
  • Working closely with clients and their informal supporters/stakeholders to identify their needs, personalised support, and ensure they are linked with appropriate services
  • Working closely with allied health professionals and government bodies such as (DFFH, NDIA, Justice Department)
  • Promoting and fostering independent decision-making, including choice and control
  • Focusing on supports to build client's skills and direct their life, while you connect them to allied health professionals
  • Coordinating and liaising with service providers, and reviewing goals, and jointly determining new ones with clients
  • Organising care team meetings to foster each client’s support needs
  • Writing reports to NDIS to support each client’s plan review
  • Collaborating with other services providers to support and build each client’s capacity to achieve their goals

Skills and Experience:

  • Driver’s licence
  • Strong administration skills
  • Minimum of 12 months experience
  • Strong can-do attitude and initiative
  • CPR, First Aid & NDIS Worker screener check
  • Excellent problem solving and multi-tasking capability
  • Higher education within the mental health/disability seminars would be admirable
