As the Coordinator of the Key Worker Team within the Southwestern Sydney (SWS) pilot hybrid child and family health hub, you’ll play a critical role in supporting young parents experiencing vulnerabilities. Your leadership will ensure that families receive the integrated care they need—linking them to early childhood development, education, mental health, and family support services.
This role is a unique opportunity to drive innovation and collaboration, working in partnership with Karitane and the SWS Local Area Health District to develop and implement holistic support pathways for vulnerable young parents.

You are a passionate and experienced leader in the child and family sector, with a strong background in casework, case management, and service coordination.
