About the opportunity:

As the Coordinator of the Key Worker Team within the Southwestern Sydney (SWS) pilot hybrid child and family health hub, you’ll play a critical role in supporting young parents experiencing vulnerabilities. Your leadership will ensure that families receive the integrated care they need—linking them to early childhood development, education, mental health, and family support services.

This role is a unique opportunity to drive innovation and collaboration, working in partnership with Karitane and the SWS Local Area Health District to develop and implement holistic support pathways for vulnerable young parents.

Your leadership will ensure that our Key Worker Model is embedded, effective, and making a real difference.

Key Responsibilities:

As the coordinator, you will:

  • Lead & Inspire – Guide a team of Key Workers, ensuring they have the training, support, and supervision to deliver high-quality, impactful work.
  • Shape Best Practice – Develop and implement a robust practice framework, intake processes, and referral pathways to ensure families receive the right support, at the right time.
  • Build Strong Partnerships – Work closely with Karitane, SWSLHD, Sonder, and key community stakeholders to strengthen service integration and accessibility for young parents.
  • Drive Innovation – Develop data collection methods and evaluation systems that help shape the future of the Key Worker Model.
  • Advocate for Families – Ensure that the voices of children, young people, and families are heard and valued in every aspect of service delivery.

Your Qualifications & Experience

You are a passionate and experienced leader in the child and family sector, with a strong background in casework, case management, and service coordination. You thrive in collaborative environments, are driven by social justice, and have the vision and expertise to lead innovative programs that make lasting change.

  • A Bachelor’s degree in Social Work, Psychology, or a relevant field
  • 5+ years of experience, including at least 2 years supervising teams.
  • Strong knowledge of child protection legislation, risk assessment, and safety planning.
  • Experience working with culturally diverse families and developing culturally responsive services.
  • A commitment to fostering an inclusive, supportive workplace for all staff and clients.
  • A current NSW Driver’s Licence & NSW Working with Children Check.

  • Knowledge of the SWS service system and strong stakeholder relationships in the region.
  • Experience supporting vulnerable young parents and understanding their unique challenges.
