About the role

The Support Coordinator will work with individuals living with a disability and support their access to a range of form and informal community based supports. You will demonstrate a commitment to providing choice and control for participants; furthering individual goals and capacity building.

This role will see you:

  • Provide assistance to strengthen participant’s ability to understand and work within the NDIS framework and to coordinate and direct supports within their plan
  • Support Individuals to map out their NDIS budget including providing provisions for service delivery, and NDIS reviews
  • Assist individuals to strengthen their capacity to establish networks, maintain support services, linkages and relationships

Skills & experience

  • A relevant tertiary or Diploma qualification
  • The ability to manage and prioritise competing demands
  • Extensive experience working directly with customers with significant and diverse disabilities
  • The ability to work autonomously and cooperatively as a member of a team
  • Demonstrated ability to identity, measure and report on outcome’s
  • Establish and maintain positive professional relationships with a variety of key stakeholders
  • Knowledge of NDIS Act 2013, Disability Act 2006 and other relevant legislation
  • Current driver’s license
