WA Primary Health Alliance oversees the strategic commissioning functions of the Perth North, Perth South, and Country WA Primary Health Networks (PHNs). The WA PHNs are three of 31 PHNs established by the Australian Government nationally to increase the efficiency and effectiveness of medical services for the community. We work across the WA healthcare system with doctors, allied health professionals, Area Health Services, and service providers to improve the coordination of care for people who are at risk of poor health outcomes.

Due to the nature of our funding and prescribed activities, WA Primary Health Alliance often operates in a dynamic and changeable environment. Adaptability and flexibility are therefore necessary to respond appropriately to changes as they occur.

The role

Consistent with WAPHA’s strategic priorities and Commonwealth guidance, and guided by WAPHA’s Strategic and Operational Plans, Place Based roles facilitate planning, co-ordination and alignment across community, relevant agencies, and service providers to enable the integration of local services and activities.

Place Based roles enable integration at a local level through working with health care agencies and providers in an intentional, coordinated way to maximise health outcomes, cost efficiencies and improve the experience of individuals accessing services and clinicians providing them. These roles ensure WAPHA can both build and align the capacity and capability of existing service provision in a specific location. This work will not be standardised across all regions and will be reflective of local community and context.

Applying local presence, relationships and contextual knowledge, Place Based roles are empowered to inform, support, integrate and communicate the work of WAPHA’s Strategy and Engagement, Commissioned Services and Primary Care Innovation and Development teams.

The Regional Integration Manager will coordinate activities within their designated regions that support successful delivery of:

  1. System and service integration
  2. Commissioning of primary health care services and program implementation
  3. General practice support
  4. Disaster and emergency planning and response
  5. Primary Care workforce strategic planning

This role will be offered as an on-going position on a full-time basis and offers the flexibility to work from home and office. 2 full-time roles available within the PHN's Kimberley & Wheatbelt Region.

Key accountabilities of this role include:

  • System and Service Integration- Integration of general practice and WAPHA commissioned services, programs, pilots, activities, and initiatives into the local health system
  • Commissioning of primary health care services and program implementation- Local stakeholders are engaged in consultation/co-design and commissioning of WAPHA services, programs, pilots, activities, and initiatives
  • General Practice Support- Embedding WAPHA’s primary care and population health programs and initiatives in Place, enabling linkage between HSPs, local commissioned services providers and general practices and providing implementation support to WAPHA teams.
  • Disaster & Emergency Planning & Response - WAPHA staff are engaged in disaster and emergency planning and response management, representing, and integrating primary care at system level at key local forums.
  • Primary Care Workforce Strategic Planning- WAPHA’s identified workforce initiatives are viable and sustainable in the context of the place.

About you

To be considered for this role, candidates must meet the following selection criteria:

  • High level understanding of the health system, both in WA and on a national scale including current trends in health policy reform and health care funding programs
  • Strong experience co-designing services with effective and sustainable results.
  • Strong interpersonal and stakeholder management skills, with group/meeting facilitation and presentation experience
  • A demonstrable understanding of the complexities of primary health care and working with partnership organisations to achieve common goals in the private, public and NGO sectors
  • Experience working in a place-based setting
  • For Country specific Regional Integration Manager should have demonstrated experience in working in rural and remote settings and be located in the region.
  • Experience working with culturally diverse groups, particularly Aboriginal and Torres Strait Islander peoples
  • Exceptional communication skills – both written and verbal
  • Adaptability to work in a dynamic, flexible, and fast-paced environment.

Preferred Knowledge and Skills:

Understanding Their Place

  • Knowledge of commissioned activity in a location (by WAPHA and others)
  • Understanding of the opportunities and barriers for integrating services, programs, and general practice in the location
  • Knowledge of service gaps and duplication in the location
  • Awareness of opportunities for greater efficiencies in the location
  • Awareness of local health priorities
  • Awareness of social determinant factors in a location
  • Awareness of workforce constraints
  • Understanding of pathways and current blockages
  • Awareness of existing local partnerships and collectives
  • Understanding of political and social contexts within location
  • Possess well established networks within location

Facilitate Partnership Approaches to Promote Integration

  • Be conversant in the different types of health integration
  • Understanding of WAPHA’s role and level of influence regarding local integration
  • Understanding level of connectedness between local GPs and other services in location
  • Awareness of the logistical barriers (i.e. time, resources, scope) for local engagement
  • Awareness of levers / opportunities to support local engagement

Promote Effective Local Governance

  • Specialist understanding of inter-organisational governance processes
  • Understanding barriers to and levers of collective governance
  • Awareness of existing governance mechanisms at a local level
  • Ability to influence others and move toward a common vision or goal
