the opportunity

We are currently seeking a dedicated and compassionate NDIS Support Coordinator / Recovery Coach to join our team. You will be responsible for assisting participants in developing and implementing their NDIS plans. You will work closely with individuals with disabilities, their families, and relevant stakeholders to ensure the provision of high-quality support services.

Responsibilities:

  • Conduct initial assessments to determine participant needs, goals, and aspirations
  • Collaborate with participants to develop person-centred NDIS plans
  • Provide guidance and support to participants in choosing appropriate service providers
  • Complete referrals
  • Coordinate and monitor the delivery of support services
  • Budget for and monitor participants' funding
  • Advocate for participants' rights and ensure they receive the necessary support and funding
  • Liaise with a range of service providers, community organisations, and government agencies
  • Maintain accurate documentation and records
  • Provide regular progress reports to participants and their families
  • Effectively and independently manage your own time

Qualifications and Skills:

  • Relevant qualification in social work, psychology, disability services, or a related field
  • Strong interpersonal skills to engage with diverse stakeholders and develop positive relationships
  • An understanding of a trauma-informed approach to practice
  • Ability to prepare reports and notes in clear and concise language
  • Strong organisational and time-management skills
  • Ability to be flexible in a changing environment
  • Enthusiasm and passion for providing quality support to participants and families
  • Ability to work independently and as part of a team

Remuneration:

  • $61.35 per billable hour
  • Casual agreement
  • Flexible hours, minimum 20 hours per week
