<h3>About the program</h3>
<p>The Intensive Therapeutic Care &ndash; Significant Disability (ITC-SD) service provides therapeutic care to children and young people who have been diagnosed with a significant disability in statutory Out-of-Home Care. This program has a strong focus on facilitating and supporting young people with their journey to recovery from trauma with a team-based approach to providing individualised, person-centred care. We work in collaboration with Wesley Disability and other stakeholders to deliver therapeutic care within a residential setting.</p>
<h3>About the role</h3>
<p>As the Case Manager, you are responsible for providing high quality and effective case work services for young people in Intensive Therapeutic Care. You will plan for and achieve permanency and identified case plan outcomes for children/young people.</p>
<p>Wesley Dalmar is seeking a Case Manager to join our Intensive Therapeutic Care &ndash; Significant Disabilities team based in Penrith. The position offers challenging and rewarding opportunities to work with children, families, and the Wesley Disability team. We&rsquo;re seeking highly motivated professionals who are passionate about making a difference through working with young people in residential care to provide an essential case management service.</p>
<p>Expect to learn skills in: navigating the NDIS, leaving care planning, communication, child protection, risk assessment, building relationships with young people, therapeutic casework, and many others.</p>
<p>The agency provides regular supervision and support for all staff and opportunities for professional development.</p>
<h3>Essential criteria</h3>
<ul>
<li>Bachelor&rsquo;s qualification(s) in Social Work, Social Welfare, Psychology, or related fields</li>
<li>Knowledge, skills and experience working with children and families</li>
<li>Demonstrated ability to write reports to a professional standard</li>
<li>Ability to work independently and as part of a team</li>
<li>Ability to mediate under pressure to resolve differences using a strengths-based approach</li>
<li>Understanding of the National Disability Insurance Scheme</li>
<li>Flexible to work some evenings and weekends</li>
<li>Current unrestricted driver&rsquo;s licence</li>
<li>Current Working with Children Check and satisfactory criminal history record check.</li>
</ul>
<h3>Desirable criteria</h3>
<ul>
<li>Experience in Out of Home Care</li>
<li>kKnowledge of the Permanency Support Program (PSP)</li>
<li>Working knowledge of the various legislation&rsquo;s that govern Out of Home Care in NSW.</li>
</ul>
<h3>Remuneration</h3>
<p>Salary and employment conditions are in accordance with the Social, Community, Home Care and Disability Services Industry Award 2010, Grade 5, with generous salary packaging available in addition to super and a fully maintained vehicle for business and personal use.</p>
