About the opportunity

This full time, maximum term opportunity for an Assessment and Planning Officer is to join our Carer Services Program. The program delivers a range of support services to carers who provide unpaid care and support to a family member or friend, who is frail due to age or has dementia, a disability, mental illness, terminal illness or chronic health condition.

As an Assessment and Planning Officer, you will undertake assessments to identify the specific needs associated with the caring role and provide information, advice and individualised support to carers. The Assessment and Planning Officer will assist with planning and coordinating an appropriate service response which may include short-term case management, respite, carer coaching and counselling, peer support and referral to other services where necessary.

In addition to a base salary commencing at $43.08 p/hr + superannuation, you will have access to:

About you

You will have a tertiary qualification in social work, health, welfare, community services or a related discipline.

You will also bring:

  • Demonstrated knowledge or experience in the provision of care to the elderly, young people and/or people who have a disability or mental illness.
  • An understanding of the issues and challenges experienced by carers.
  • Ability to work autonomously and as part of a collaborative team.
  • Strong engagement, care planning and assessment skills.
  • Excellent organisation, time management and communication skills.
