Uniting’s Carer Support Services Program delivers a range of support services to carers who provide unpaid care and support to a family member or friend, who is frail due to age or has dementia, a disability, mental illness, terminal illness or chronic health condition.
As an Assessment and Planning Officer, you will utilise the Carers StarTM conversation tool to understand and assess the specific needs associated with the caring role and develop goal orientated care plans in conjunction with carers. You will assist with planning and coordinating an appropriate service response which may include short-term case management, respite, carer coaching and counselling, peer support and referral to other services where necessary.
