We are seeking a dynamic and detail-oriented Intake Coordinator to join our team. In this pivotal role, you will be responsible for managing the intake of referrals, conducting placement matching, and overseeing case management and assessments. Your contributions will play a critical role in ensuring compliance with audit and quality standards while fostering positive and collaborative relationships with volunteers, stakeholders, and funding bodies.
As the Intake Coordinator – Out of Home Care, you will:

You will have a Bachelor level degree in Social Work or a related discipline. A Certificate IV in Training and Assessment as well as previous home-based care case management experience would be advantageous. You will also have:
