The Journey to Social Inclusion (J2SI) is an innovative and evidenced-based case management service for people experiencing long-term homelessness in Central Highlands, Victoria.
This program differs from other homelessness services as it provides the time needed for people to access their permanent homes and have an improved quality of life and increased community connection.
As an Intensive Case Manager, you will provide long term case management and service coordination with low individual caseloads who have experienced chronic homelessness.
In this role, you will manage a caseload in an outreach capacity with the aim to provide a supportive and effective service to achieve client goals, program outcome, referral and advocacy support and service coordination.
Our work isn’t easy, but it matters. Work with people who care as much as you.
