About the opportunity

This full time, ongoing opportunity is to join our Carer Services Program. The program delivers a range of support services to carers who provide unpaid care and support to a family member or friend, who is frail due to age or has dementia, a disability, mental illness, terminal illness or chronic health condition.

The Senior Assessment and Planning Officer (Senior APO) is responsible for:

  • Providing support to the Carer Gateway team by providing secondary consultation, guidance and mentorship to the Assessment and Planning Officers.
  • Managing assessments, developing goal directed care plans and providing coaching for carers with particularly complex presentations.
  • Supervision with the Carer Coach to support the delivery of consumer-centred coaching with carers across the service. involved in the facilitation of in-person peer support groups with carers.

About you

You will have a tertiary qualification in social work, health, welfare, community services or a related discipline. You will also bring:

  • Advanced skills in assessment, care-planning, referrals, and service navigation.
  • Previous leadership experience in a similar role.
  • Knowledge of the community care sector, including the challenges faced by carers of people with a disability, chronic illness, mental health condition and the aged.
  • Proven ability to foster and maintain relationships with internal and external stakeholders and to relate to and work effectively with a diverse range of individuals and communities.
