About the Role

The Active Recovery Team – ART Perth North, Pilot Project is a recovery focused collaboration between North Metropolitan Health Services (Adult Mental Health Service) and Womens Health and Family Services. The ART Project provides multidisciplinary co-ordinated support and treatment for individuals with severe and/or persistent mental health issues; have multiple treatment and support needs; and would benefit from recovery focused intensive psychosocial support to keep them well in the community.

Reporting to the Team Leader, Active Recovery Team (ART) Counselling & Support Services (CASS), your role is to provide emotional and practical support, case management and counselling to consumers on an individual and group basis (includes face to face, telephone, and outreach). You will also assist with identifying their needs for wellness, options for recovery and support to navigate their way through community services and supports.

Full-time or part-time position available split across our Joondalup and Wanneroo offices.

Job Requirements

To be successful in this role, you will undertake the clinical assessment of clients and potential clients who will often present in crisis. Provide advice (and redirection where required) to clients, their families and carers, and other service providers (including external and internal referrers) for people who may not need hospital admission but need intensive support to engage in community Services, networks, and programs. You will also Provide one-to-one counselling, advocacy and support to participates and facilitate Mental Health and AOD support groups, SMART recovery groups, and information sessions to consumers accessing the ART service when required.

Essential

  • Qualified in one of the following disciplines: Mental Health or Alcohol and Other Drugs (AOD) Counselling, Social Work, Psychology or equivalent.
  • Demonstrated experience as a mental health and/or AOD counsellor.
  • A good understanding of recovery-oriented practice and available AOD and Mental Health services in the community with a demonstrated ability to support clients to access services, groups, information, and other supports to strengthen and support their recovery.
  • Demonstrated ability to build partnerships with government and non-government health and welfare service providers to pursue practical strategies to improve service delivery to consumers.
  • Ability to write comprehensive case notes, engage in case conferencing and case formulation and write reports when required.
  • Clinical and problem-solving skills with a demonstrated ability to work alongside people with multiple needs and complex trauma using a case management approach to treatment.
  • A demonstrated understanding of diversity and inclusion, access and equity, social justice and multiculturalism in Australia and cultural sensitivity in relation to best practice.
  • Competent in using Office 365 products.
  • Valid Western Australian Driver’s License, Satisfactory Criminal Record Check, and COVID-19 vaccinations.
