About the Pharmacotherapy Support Lead role

As Pharmacotherapy Support Lead at YourCH, you'll connect people who use drugs with care, coordinate treatment, support recovery, and improve service access. Working across Harm Reduction and medical teams, you’ll help deliver inclusive, client-focused health support.

Reporting to the Harm Reduction Coordinator, this is a fixed term part-time role (0.6 FTE). Offices located in the Northern Suburbs.

Your Passion and Commitment will see you:

  • Deliver high-quality AOD and administrative services
  • Collaborate with Addiction Medicine NP and GPs to plan care and identify clients for EPC
  • Coordinate care, ensuring regular client contact and case conferencing
  • Liaise with pharmacies, AOD workers and external services like PAMs
  • Manage financial admin for the pharmacotherapy program.

To succeed in this role you will have:

  • Relevant qualifications and/or experience in the health, welfare, community services or alcohol and other drugs (AOD) sector
  • Drivers Licence
  • Strong understanding of AOD, chronic disease and complex care based on best practice guidelines
  • Commitment to harm reduction principles with a proven ability to apply them practically
  • Effective communicator with diverse social and cultural backgrounds
