About the Diabetes Educator

As the Diabetes Educator you'll work within a multi-discipinary Allied Health program to provide high quality and comprehensive Diabetes Education and support that is especially tailored to reaching priority population groups in our community.

Reporting to the Coordinator - Dietetics, Diabetes Education and Community Nursing, the Diabetes Educator is a part-time (0.4FTE) ongoing position.

Your passion and commitment will see you:

  • Offer person-centred comprehensive care management and support with individuals and groups, on-site and in community settings.
  • Apply a model of care that ensures that chronic disease self-management, contemporary health promotion, and wellness and reablement principles are applied through a social model of health.
  • Build relationships to facilitate access to YourCH services for clients with diabetes, including with the broader Allied Health team, YourCH Programs (including Primary Care, Community Programs, Gender Affirming Care), acute setting partners and other agencies/services (e.g. Austin Diabetes Care-management and Assessment Service) .

To succeed in this role you will have:

  • Post Graduate qualifications in Diabetes Education and evidence of credentialing as a Diabetes Educator.
  • Current AHPRA Registration.
  • At least 3 years clinical experience in providing well-developed effective diabetes education services.
  • Demonstrated clinical skills providing diabetes education for a range of population groups.

As a YourCH employee in this role you must have:

  • A Victorian Working with Children Check (employee).
  • A Satisfactory Police Check (paid by YourCH).
  • Proof of COVID-19 Immunisations (2 Initial doses and at least 1 Booster).
  • NDIS Work Screening Clearance.
