About the role

An opportunity exists for an experienced Community Service Worker who can go above and beyond when it comes to delivering services to our Beneficiaries. You will be confident and experienced in working from a strengths based approach. You will be known for your excellent interpersonal and relationship-management skills. You will be proactive and are looking to join an organisation that is dedicated to enhancing lives.

This position is part-time (35 hours per week), and will have you working from home with a requirement to conduct visits with our beneficiaries in the Blue Mountains, Nepean, Hawkesbury and Parramatta Divisions.

Key Responsibilities

  • Conduct regular home visits to Legacy beneficiaries located in the North West Sydney metro areas, with use of your own private motor vehicle – there may be requirement to visit other areas around the Sydney metro (travel expenses reimbursed)
  • Advocate for and maximise the independence of the people that we support by providing quality service referrals in a person centred approach
  • Assist in the development, implementation and review of case plans (where required)
  • Assist the people we support to participate in and access community facilities / events
  • Update beneficiary records and file notes as required in the applicable database
  • Completion of all documentation within required timeframes
  • Attend all relevant meetings, forums and training

Skills and Experience

  • Minimum Cert IV in community services, welfare or aged care
  • Minimum 2 years’ experience working in the Community Services sector
  • Sound knowledge of services within the aged care sector such as DVA, My Aged Care and NDIS
  • High level of interpersonal / communication skills and proven ability to engage in and build trust and rapport with internal and external stakeholders
  • Highly developed organisational and time management skills to effectively organise workload, priorities and meet deadlines
  • Proven ability to work independently and as part of a team
  • Current NSW Class C driver’s licence (unrestricted)

Highly Regarded

  • Current First Aid Certificate or willingness to obtain
  • Previous experience working in the community services sector
  • Previous experience with CRM and online case management tools
  • Knowledge of trauma informed practice, person centred practice and a strength based approach to service delivery
  • Experience with Microsoft Office suite of products including Word, Excel, Outlook and Teams

Applicants will be required to undertake a police check, and working with children’s check and must provide evidence of a current driver’s licence and a roadworthy motor vehicle as these are essential for this position.
