Your new role

As a HARP CHF Nurse you will be responsible for:

• Provide high standard of clinical care for clients with Chronic Heart Failure in accordance with best practice guidelines including CHF specialist assessment, education, care planning and referrals.
• Improve self-management and health outcomes through provision of education on anatomy and physiology of Cardiac conditions, symptom recognition, medications, exercise and exercise tolerance; advanced clinical assessment and assistance with exacerbation management.

• Demonstrated experience in managing clients with CHF through effective assessment, care planning, case conferencing, review and escalation of care for clients.
