About the role

Mind is seeking a Community Mental Health Group Facilitator to join our Rockingham (Kwelena) Step Up Step Down service. This facility provides a short stay 24/7 residential service, supporting opportunities for skill development, enhanced social relationships, and community access.

Working closely with clinical and community partners, this recovery and strengths based service supports people to successfully transition from hospital back to their home in the community (Step Down) and prevents hospital admissions for people who are becoming unwell (Step Up).

Reporting to the Service Manager, you will facilitate group work, provide comprehensive assessment of client need, individual recovery plan development and review, delivery of individualised client support, short-term self-management and carer support.

Duties

  • Deliver high quality, recovery oriented, direct support to individual customers
  • Develop and deliver groups and learning activities for clients experiencing mental ill health
  • Engage with customers to fully understand their need for assistance
  • Work with families, carers, and local service providers
  • Contribute to the day-to-day operations of the residential service

What are we looking for?

  • Tertiary qualifications (minimum Cert IV in Mental Health, Psychology, Social Work, Occupational Therapy or other health related field as designated by Mind)
  • Previous experience delivering group sessions; working in a similar role and recovery orientated frameworks
  • The ability to build relationships with internal and external stakeholders
  • Demonstrated ability to plan and prioritise to meet customer service delivery requirements
  • Ability to adapt to changing priorities flexibly, proactively and with ease
