The Yandina Transitional Housing and Mental Health Accommodation Pathways at Discharge (MHAPD) outreach services sit within Mind’s Sub-Acute program and operates as a client centered framework providing services to clients with severe mental illness. These services provide recovery based interventions aimed at successfully supporting clients to address and overcome barriers that contribute to homelessness or increased risk of homelessness.
Working autonomously with an empathetic team, we are seeking two Community Mental Health Practitioners for 2 year fixed term, full time or part time (23 hours/ week) opportunities. Based at home but working in the Box Hill and Inner Eastern suburbs you will work with clients in the community to access accommodation services and support mental health recovery.
