We are currently on the hunt for a Senior Support Coordinator to join our small team to lead the effective implementation of MDWA’s Support Coordination service. We need an experienced, passionate and empathetic person who will support our community living with neuromuscular conditions and disabilities.

About this role

This pivotal role will guide MDWA into Support Coordination service delivery through the review of existing resources and materials, recommendations for ICT support, training and development of any support staff, and implementation of the service when we are ready to deliver.

As a Senior Support Coordinator, you will work with our community, their families and support networks, and across organisational teams to help them to achieve the most out of their NDIS plan.

You’ll identify their goals and support them in accessing the services they need, always ensuring their choice, control and independence is at the core of everything you do. You will assist them to understand, implement and coordinate services for the length of their NDIS plan.

This role is part time (0.4 FTE, 7.5 hrs a day on nominated days Monday through Friday) on a fixed term contract until the end of September 2023, with the likelihood of extension.

Key responsibilities

First up, we’ll get you to review the current resources, materials and tools developed to deliver MDWA’s Support Coordination services and make recommendations for improvement.

You’ll review our CRM and let us know how you think it can work in regards to integration of NDIS support modules.

You’ll recommend or develop tracking and reporting tools for efficient service delivery.

We’ll want you to work with the Community Services Manager and our Communications team to promote the Support Coordination service.

You’ll train and support staff directly involved with MDWA’s Support Coordination service including portal training, service bookings and onboarding.

It’ll be essential that you ensure MDWA’s Support Coordination Services meet the appropriate NDIS legislation, guidelines and funding frameworks.

You’ll work with internal programs to ensure advocacy and referrals in a safe and compassionate environment.

There will be times when you’ll work alongside other service providers in an inter-agency case management approach.

It’ll also be your role to oversee and update risk assessment and safety plans.

You’ll do some report writing and record keeping, contributing to funding reports.

And finally, you’ll work to deadlines and prioritise work plans daily.

Essential criteria

These items are a ‘must-have’:

  • Minimum three years’ experience working within the NDIS as a Support Coordinator or team leader
  • A good understanding of disability support services available in WA
  • You understand the challenges faced by people living with disability and can advocate on their behalf with a commitment to their choice and control
  • Knowledge of the resources/tools required to run a Support Coordination service
  • Experience adhering to organisational contractual obligations including outcomes-based models and client measurement tools
  • Knowledge and skill in using Risk Assessment and Safety Plans
  • A comprehensive understanding of person-centred planning, working within a Case Management framework and developing and reviewing support plans
  • Expert knowledge of referral pathways and community services
  • A complete understanding of the appropriate relationship boundaries with clients and other service providers and the ability to work within the constraints of the job role
  • You can adapt with ease, working as a member of a team, leading a team, and working independently on program outcomes whilst exercising good judgement and initiative
  • Problem-solving of moderately difficult issues using knowledge, judgement and organisational skills

Desirable criteria

We’d love you to have these, but they aren’t essential:

  • A qualification within the disability or social services sector
  • Understanding of the needs of people specifically living with neuromuscular conditions
  • Networks and contacts within the health and disability sector that can be leveraged for the benefit of the organisation
  • Experience working with ICT systems in health & disability sectors
  • Financial management skills
  • You're a non-smoker
