We are seeking an enthusiastic and experienced Case Manager to deliver high quality case management support services to young men, women, and families, including those with accompanying children who are at risk of or experiencing homelessness This position is based in across two sites in St Kilda at the Upton Road youth refuge and Fitzroy North at the Tranmere Street Refuge. Youth Services is committed to working in partnership with young people to address the issues contributing to their unique experience of crisis & homelessness, with a particular focus on assisting young people to:
Case managers are expected to carry a mixed caseload of up to 12 young people working across the following program areas.
Youth & Family Outreach Program (YFOP) - supporting young people to live independently in the community, with a particular focus towards supporting young women and accompanying children.
Upton Rd & Tranmere St Youth refuges - supporting young people experiencing significant crisis and homelessness in supportive & therapeutic environments at St Kilda and Fitzroy locations.
Youth Private Rental Access Program (YPRAP) - assists young people prepare for and actively engage with private rental and share rental market, through a combination of support, education and financial brokerage.
Refuge Transitions Program - provides a continuum of support for young people leaving the youth refuge to ensure that independent living options are successfully maintained in the community.
Youth Services staff work within a trauma & psychologically informed practice environment, all staff are expected to work creatively towards ensuring that the support needs of all young people are the first priority.
