The opportunity to work in an exciting, innovative and fast paced community aged care environment. Looking for a passionate and experienced Program Manager to oversee the operational management of Consumer Directed Care (CDC) Home Care Packages program staff and service delivery.

KEY RESPONSIBILITIES INCLUDE:

  • Identify any changes and review policies and procedures that directly impact upon Community Aged Care
  • Monitor contract compliance, quality and continuous improvement
  • Manage the financial budgets of the operational team
  • Appropriately manage performance of Community Aged Care programs including coaching support to direct reports, regular informal and formal performance feedback and reviews, and support for relevant training and development
  • Ensure the portfolio is staffed appropriately with quality team members who are committed to high standards of performance and service.
  • Establish and maintain effective and meaningful relationships with new and existing internal and external stakeholders

To be successful in the role you will have:

  • Degree in Social Work / Allied Health / Nursing or equivalent, or extensive experience working in a similar role (essential)
  • Current National Police Clearance – Aged Care (essential)
  • Current SA Drivers Licence (essential)
  • Applicants MUST have community aged care coordination/management (or similar) experience in Consumer Directed Care Home Care Packages (CDC)
  • Experience working within Commonwealth funded aged care programs
  • Significant experience managing and leading teams in the human services/health sector
  • Experience undertaking case management and caseload reviews within a community services, disability or aged care setting

You will also demonstrate a positive and professional "can-do" attitude, excellent verbal communication skills, with the ability to work equally well as a member of a team or working alone.
