About the role

Part-time (0.8 FTE) fixed term contract until 30 June 2025. This role services regional South Australia, location can be flexible across the state. Reporting directly to the Team Leader Client Services, you will be responsible for:

  • Delivering high quality Occupational Therapy service to people diagnosed with dementia and/or their families and care partners.
  • Undertaking screening and assessment of clients to ensure appropriate to the client’s environment, goals and presentation.
  • Developing goal directed, person-centered interventions, empowering clients through adaptations to domains impacting their ability to live as well as possible with dementia.
  • Provide holistic and comprehensive recommendations to clients and their care partners.
  • Refer clients to service providers as appropriate internally and external to Dementia Australia.
  • Ensure key outcomes and deliverables relating to funding agreements are consistently met.
  • Supporting increased community capacity through building collaborative partnerships with other service providers and external stakeholders as appropriate.
  • Frequent weekly intrastate travel, including some overnights.

About you

To be successful in this role, we are looking for someone with the following:

  1. BSc (Occupational Therapy) or equivalent, with a minimum of 5 years’ experience.
  2. Registration with Australian Health Practitioner Regulation Agency (AHPRA).
  3. Postgraduate studies in Palliative Care and/or Dementia Care will be highly regarded.
  4. Willing to undertake a WWCC (Working with Children Clearance) prior to commencement.
  5. Collaborates with others to remove barriers and break down silos.
  6. Achieves results through a commitment to quality, planning and efficient resourcing.
  7. Continually develops and applies specialist and technical skills.
  8. Provides service excellence by delivering client focused services.
