At ACSO, we look for people who believe in second chances, who share our values and are passionate about supporting forensic clients to re integrate back into the community. If all of this sounds like you, read on!
The Australian Community Support Organisation (ACSO) is a vibrant, values driven NFP, focused on partnering with Government and the community to co-design services that create real social change. We are an organisation with a bold vision to create a safe and inclusive community where prison truly is the last resort. Our purpose is to strengthen the wellbeing of our communities by advocating for and delivering services which divert people away from the justice system.
The Australian Community Support Organisation recognises the right to a safe and inclusive service without bias. ACSO is committed to the equitable treatment of its participants, employees and partners. We believe in humanity and celebrating the diverse voices of our community through leadership, practice and policy design, to honour and embrace diverse traditions, cultures and experiences of those we support and work alongside.
The Head to Health Intake Worker will deliver a 'whole of person' assessment to assist in defining the appropriate 'level of need'. The role involves navigating timely access and warm transfers to connect people to other services specific to the region including consumers, carers and family members.
The Intake Worker will also develop and maintain effective relationships with stakeholders (such as GPs, NGOs, Local Hospital Networks and community-based organisations) to improve service response for clients and promote the program to potential referrers. This will support the intake team to maintain linkages and referral pathways, and to connect to services and provide for a consumer-centred focus.
Head to Health is an Australian Government initiative created to ensure clients are receiving the right level of care, at the right time, using a hybrid intake and referral model to support appropriate client allocation. Head to Health intake and referral processes are intended to complement other intake approaches already provided in the community.
Head to Health will provide a highly visible and accessible entry point to services for people experiencing psychological distress. The intake process provides a no wrong door approach, ensuring all referrals are supported to engage with an appropriate service that meets their needs. Head to Health uses a standardised national referral form and intake process to ensure consistency.
If you think you'd be a great fit - we'd love to hear from you!
