DUTIES:

  • Provide short term, episodic support to eligible clients needing to engage with community and other services at key stages of the care pathway, as reflected by the key elements of the Service Coordination framework (initial contact, initial needs identification, and referral).
  • Provide information about the range of services to targeted diverse communities and individuals within them.
  • Work collaboratively with services to facilitate improved client access and support.
  • Promote better practice in service delivery responses to meet the needs of diverse communities and the individuals within them.
  • Develop effective links and establish trust specifically with communities and individuals from CALD backgrounds.
  • Use an active service model (strengths-based/capacity-building approach) to needs identification and service provision that recognises and builds on client’s existing skills and resources.
  • Use strategies to empower eligible clients, and their carers, and build their confidence in relation to access and use of services.
  • Contribute as required to the preparation and submission of data and statistical reports and returns, and ADEC management reports.
  • Undertake other administrative tasks as required.
  • Other duties as specified by the Aged Care Unit Coordinator.

RESPONSIBILITIES:

  • Attend and contribute to ADEC Staff meetings and when rostered, Board of Management meetings.
  • Attend and contribute to team meetings.

KEY PERFORMANCE INDICATORS:

The S3 Officer’s performance will be measured against the following criteria:

  • Attainment of target outcomes set by the Aged Care Unit Coordinator.
  • Level of liaison and engagement achieved with ethno-specific and multicultural providers, and generalist providers.
  • Consumer satisfaction with services provided.

KEY SELECTION CRITERIA:

Essential:

  • Appropriate qualification in Community Services, Community Development or Aged Care.
  • A broad understanding of diversity issues with people over 65 years as they relate to individuals and their engagement with community care services.
  • Sound knowledge of Home Care Package and Community Home Support Programs.
  • Experience of direct service provision within a Service Coordination context, including the key elements of initial contact, initial needs identification, assessment, care planning and the Service Coordination, and relevant service standards.
  • Sound understanding of the Aged Care Act, Aged Care Quality Standards and Quality of Care Principles Demonstrated ability to deal sensitively and diplomatically with a diverse range of individuals and service providers, to advocate on behalf of clients, and to empower people to self-advocate.
  • Highly developed verbal communication skills, demonstrated ability to write in a way that is clear, appropriate and readily understood by the target audience, and an ability to produce community information.
  • Previous experience in the use of electronic referral and care planning systems and electronic service directories.
  • Hold a valid and current licence to drive a motor vehicle, issued by the appropriate authority in the jurisdiction in which the holder will drive, and appropriate to the type of vehicle to be driven.

Desirable:

  • Certificate in Community Services, Community Development or Aged Care.
  • Relevant post-secondary qualifications and experience relating to an area of health and/or community services practice such as Disability Studies, Aged Care, welfare studies, case management, and/or social work.
  • Previous experience of direct service provision in the CHSP sector, preferably in a care manager role involving assessment, advocacy, care coordination, and case management.
  • Previous experience of working with people with disability and carers from ethnic communities.
  • Ability to accurately collect statistics, record data, and maintain records.

AWARD AND CLASSIFICATION:

  • All ADEC employees are employed under the terms, conditions and entitlements of the Modern Award – Social, Community, Home Care and Disability Services Industry Award 2010 (the Award); and the National Employment Standards (NES).
  • The classification for this position is Level 3.
