The Care Hub is an initiative that brings together a number of organisations to form an integrated team of professionals.
The aim of the Care Hub is to provide a wraparound service and team of staff to support effective assessment, planning and intervention to help support the Children, Young People and Families through the process and ensure all needs are met.
The team will be based from a single location in Bendigo, forming a collaborative, integrated team of staff helping support those entering care for the first time.
The Community Worker will provide place-based support to Children and Young People as part of the outreach component of the Care Hubs support.
The aim of this role is to promote and support Children and Young People with engagement in the community, access to community based programs such as sport or club involvement and using this engagement to help develop important social and life skills. The Community Workers will also work with those who may need additional support to engage with school, education or employment.
