About the Opportunity

As a collaborative member of our Aged Care team, you will identify tailored care solutions to enable our consumers to achieve their goals and enhance their quality of life. Your work will involve Home Care Packages, Commonwealth Home Support Program, Transition to Care Program, fee for service work and any new programs that emerge from the Aged Care reform process.

As the Care Coordinator, you will also provide effective supervision and support to designated staff; ensure administration, data and financial records are current and support group activities, consumer forums and continuous improvement efforts.

To be successful you will have:

  • Qualifications within Allied Health, Enrolled Nurse (RN2) and/or Diploma in Community Services or
  • Demonstrated experience (minimum 3 years) experience in the Aged Care Sector.
  • Relevant experience in care coordination, care management and assessment and a demonstrated ability to provide delivery of services to the aged within their homes.
  • Experience supervising small teams with a high level of motivation and a team orientated approach.
  • Prior experience or, at a minimum, demonstrated commitment to working respectfully with Indigenous and culturally and linguistically diverse clients, communities, staff, and Aboriginal controlled organisations.
