annecto is an independent, not-for-profit organisation committed to increasing opportunities and choices for people with a disability, older persons and their carers. annecto has been around for 60 years however we have only been in Canberra for 5 years. During this time we have established a solid reputation for our high quality, personal and flexible services. Our Support team members are all directly employed by annecto.

About the position

This is an exciting opportunity to work within a small, friendly, client-focused team in an independent, not-for-profit organisation located in Mawson. As well as Case Management with our Home Care Package clients, you will also be actively involved in service promotion, providing advice on access to community-based aged care to community members and working closely with our team of wonderful direct Support team members.

What we need from you:

  • Demonstrated previous experience in client assessment and Case Management in a community setting, preferably in aged care;
  • Demonstrated knowledge of, and ability to access a range of community resources and professional services for clients;
  • Demonstrated ability to work effectively with a culturally diverse group;
  • Demonstrated ability to work through challenges in systematic and creative ways;
  • Demonstrated ability to work autonomously with a high degree of initiative and attention to detail;
  • Demonstrated ability and confidence to actively promote services to build client base;
  • Excellent communication skills, both written and verbal;
  • Proficient in the use of computer software: Word, Excel, Internet and email;
  • Demonstrated commitment to the ‘Purpose and Principles’ of annecto – the people network;
  • A current valid Australian driver’s license

What would be great to have?

  • Experience using Client Management Systems such as Techone, Alayacare
  • An understanding of the ageing process and its cultural impact;
  • An ability to speak a community language/s.
