AQA is a member-based not-for-profit organisation serving the community for 35+ years. We resource people whose lives have been disrupted by injuries, illnesses, and other events, to find their way to participate in and enjoy all aspects of life.

Established in 1987 by a group of people with quadriplegia who saw an urgent need to improve access to information, support and employment opportunities for people who had sustained a spinal cord injury (SCI).

The Opportunity

The Support Coordinator will ensure that the support is delivered in line with AQA’s values of integrity, trust, respect and excellence, and within the framework of partnership.

Lived experience is central to the work we do at AQA. Our partnership framework - and all our services - leverage the accumulated resources and knowledge of our staff, volunteers, mentors, clients and community members.

You will support people in our community to navigate the service system, and explore the service options available to them.

  • Provide Support Coordination and Case Management Services across multiple funding bodies.
  • Empower participants to exercise choice and control to access a range of formal and informal, mainstream and community-based support, by identifying opportunities for innovative service delivery.
  • Implement, monitor, and review the effectiveness of individual supports, negotiate supports as agreed, and identify solutions for any barriers to service delivery.
  • Develop and build participants capacity to continue to use and maximise their NDIS plan to achieve their stated goals after their support coordination service ceases.
  • Maintain accurate and comprehensive information by maintaining up-to-date data collection, reports and maintenance of databases in order to monitor effective service delivery.
  • Ensure the required service and billing records are accurate and maintained.
  • Optimise the use of resources available to provide quality services and outcomes.
  • Support the delivery of information sessions, pre-planning workshops, 1-1 consultations and other appropriate capacity building to support people transitioning to NDIS, as well as self and organisational development.
  • This role will require a combination of working in the office and from home.

  • Passionate with demonstrated knowledge of the NDIS and relevant legislation.
  • Creative, solutions oriented, out of the box thinker.
  • Excellent skills in communication, motivation and organisation.
  • Demonstrated capacity to build and maintain effective working relationships with internal and external stakeholders.
  • Committed to working collaboratively with your Support Coordination team, supporting each other to achieve the most out of what support coordination has to offer.
  • Able to work both independently and as part of a team.
  • Excellent interpersonal skills.
  • Excellent written and verbal communication skills.
  • Demonstrated ability to deal with confidential and sensitive matters of a complex nature.
  • Positive, solutions focused attitude.
  • Provide evidence of a COVID-19 vaccinations before commencing employment or a valid medical exemption certificate.
  • NDIS workers screening check
  • Current Working with Children Check
  • An Australian driver’s licence

Must have

  • A minimum of certificate IV/Diploma in Disability, Mental Health or Community Services.
  • At least two years of experience in the disability, mental health or community services sector.
