As a Home Support Assessor you will travel extensively across your region to conduct assessments in various areas and geographic locations. Using a holistic and client centred approach, these assessments will support our clients to continue living independently in the community by considering needs such as assistive technology, social support services and community care.
You will utilise a number of technologies, including the National Screening and Assessment Form, video conferencing, internal social media together with various software products, to support elderly clients in their home and stay connected with your team.
You will be ideally located South/South East of Sydney - for example anywhere between Surry Hills, Punchbowl, Bankstown and further South. We are open to discussion regarding part time or full time role work hours. This position will be on a 12 month fixed term contract (with potential for renewal).
