About The Role

As a key member of the ATSICHS Brisbane team, the role of Care Coordinator is to contribute, where possible, to ATSICHS comprehensive health support services to clients with the aim to achieve improved health outcomes for all Aboriginal and Torres Strait Islander people. Contribute and participate in a diverse and multi-discipline team that promotes a flourishing workplace culture and upholds the values of ATSICHS Brisbane as a long-standing Aboriginal Community Controlled Organisation.

Duties

  • Provide Care Coordination to assist in the provision of chronic disease management and promote health care and planning as per IUIH Model of Care.
  • Ensure appropriate Management and Coordination of individual Chronic Disease Care Plans and Health Checks.
  • Provide quality Aboriginal primary health care by assisting the primary health care team in effectively planning and coordinating the workload to achieve results.
  • Commit to a high level of quality management for ATSICHS through use of LogiQC quality management systems.
  • Accurate and timely collection and reporting of episodes of Care Coordination and Supplementary Services.
  • Competence in the use of clinical software systems and other technology, the internet and desktop applications e.g. outlook, Mmex, LogiQC, word, spreadsheets and telehealth software.
  • Commit to ongoing continuous quality improvement processes with annual reviews into the efficiency of clinical service provision.
  • Cultivate productive working relationships at all levels, which support the nurturing of internal and external relationships, facilitates cooperation, trust and respect, values differences and diversity and guides, mentors and focuses on the development of staff.

  • Salary of $73,358 per annum (based on qualifications and experience).

COVID-19 Requirement

  • Immunisation history showing you have received two doses of a Covid -19 vaccine is encouraged.

Required

  • Minimum Diploma in Nursing and/or Certificate IV (4) in ATSI Primary Health Care Practice
  • Full un-restricted AHPRA registration.
  • Previous demonstrable experience in relevant nursing field.
  • Demonstrated ability to meet targets and performance outcomes.
  • Ability to support a continuous improvement model for achieving outcomes.
  • Decisive listening, influencing and persuasive skills.
  • Working with Children’s Card (Blue Card) or be eligible to apply
  • Previous experience in chronic disease management.

Preferred

  • Minimum two (2) years’ experience in a primary health setting.
  • Previous experience of working with Aboriginal and Torres Strait Islander clients.
  • Previous experience in a multidisciplinary health team.
