• Position: Grade 3 Social Worker
• Location: Banksia Palliative Care, servicing the local government areas of Banyule, Nillumbik, and Whittlesea.

Role Summary:

• Provide client and family-centred care that is equitable, holistic, dynamic, and inclusive.
• Support individuals with a life-limiting illness to live their best life.
• Offer compassionate care for clients and support for families and carers, including support groups, counseling, bereavement support, and more.
• Strive for excellence, act with integrity, respect others, and strengthen through collaboration.
• Ensure quality of life, dignity in death, and support in bereavement as part of the mission.

Key Responsibilities:

• Conduct social and emotional assessments and interventions where issues impact the quality of life and well-being outcomes.
• Collaborate closely with medical, nursing, and allied health staff to deliver high-quality care.
• Liaise with external service responders and providers to achieve quality outcomes.
• Mentor and supervise junior staff and foster a positive team and workplace culture.

Qualifications:

• A four-year undergraduate social work degree or a two-year Master’s degree accredited by the Australian Association of Social Workers.

Experience:

• Preference for candidates with 5 years of community social experience.
