- Position: Grade 3 Social Worker
- Location: Banksia Palliative Care, servicing the local government areas of Banyule, Nillumbik, and Whittlesea.
Role Summary:
- Provide client and family-centred care that is equitable, holistic, dynamic, and inclusive.
- Support individuals with a life-limiting illness to live their best life.
- Offer compassionate care for clients and support for families and carers, including support groups, counseling, bereavement support, and more.
- Strive for excellence, act with integrity, respect others, and strengthen through collaboration.
- Ensure quality of life, dignity in death, and support in bereavement as part of the mission.
Key Responsibilities:
- Conduct social and emotional assessments and interventions where issues impact the quality of life and well-being outcomes.
- Collaborate closely with medical, nursing, and allied health staff to deliver high-quality care.
- Liaise with external service responders and providers to achieve quality outcomes.
- Mentor and supervise junior staff and foster a positive team and workplace culture.
Qualifications:
- A four-year undergraduate social work degree or a two-year Master’s degree accredited by the Australian Association of Social Workers.
Experience:
- Preference for candidates with 5 years of community social experience.
