The Opportunity

The program provides Complex, Intensive and General Home Based Care for children and young people who are unable to live with their own families. Most children and young people are placed in out of home care following child protection intervention. A small number are in care on a voluntary basis. Voluntary and statutory placements can be of a short or long term nature.

Key Responsibilities

As a Senior Case Manager in the program, you will use your understanding of the issues related to child development, including the impact of trauma and abuse, and will utilise this knowledge both to support children in care and to establish and maintain foster care placements.

Key duties include:

• Develop individual case plans which specify objectives of intervention
• Case management and case support services to foster carers, children, young people and their families.
• Co-ordinate and lead Care Teams.
• Work closely with the Team Leader and TCP Coordinators on program development
• Ensure completion and compliance with administrative and documentation requirements and standards.

You will also be a strong team player who is able to lead and mentor others.

 You will also have;

• Bachelor qualification in Social Work or other relevant area.
• Extensive experience in child, youth and family welfare experience, with particular experience in out of home care.
• Exceptional understanding of the legislative, policy and practice environment of out of home care sector.
• Experience in supervision of carers.
• Knowledge and experience in case management and therapeutic work.
