About the Role:

As a Case Manager for the Emotional Wellbeing for Older Persons Program, you will play a vital role in providing case management services to our elderly clients living in Residential Aged Care Facilities (RACFs) across the Central and Eastern Sydney PHN region with mental health needs through support, advocacy and referral pathways with local service providers. This assists participants to gain the skills and confidence necessary to manage their mental illness and its effects within a framework of recovery.

These positions are a Maximum Term (till June 2025), full-time role based at Summer Hill. This role will pay on SCHADs level 4.1

Key responsibilities (not limited to):

  • Conduct comprehensive mental health assessments for older adults
  • Develop personalised treatment plans tailored to each client's unique needs
  • Ensures that all the relevant case management aspects are coordinated and implemented.
  • Collaborate with healthcare professionals, caregivers, and family members to ensure holistic care and support
  • Advocate for the mental health needs of older adults within the RACF
  • Ensure that participants are involved in implementing individual recovery plans as much as possible by facilitating and encouraging them to work toward goals and increase capacity to become more independent.
  • Engage with staff of RACFs to market and promote the EWOP program and encourage their adoption of the program

About you:

  • Relevant tertiary qualifications in social work, counselling, nursing, psychology. Would particularly suit a new graduate
  • NSW Drivers licence
  • Experience working with people who have a mental illness and/or aged care
  • Excellent written and verbal communication skills
  • Valid NSW Working with Children's Check for paid employment purposes, NDIS check and certification, Police check
