Launch Housing is a secular Melbourne-based community organisation that provides homelessness services and life-changing housing supports to disadvantages Victorians. Reporting to the Program Manager - Assertive Outreach, you will have a dual focus across both the Melbourne Service Coordination and the Stonnington Zero Projects. These projects are built around the operation of effective service coordination focused on a By-Name List of persons known by local health & homelessness services to be experiencing rough sleeping homelessness in each LGA.
You will provide oversight of services to individuals identified through the Melbourne Service Coordination Project and will develop the system coordination capabilities of the Stonnington Zero Project.
You will work closely with Melbourne Service Coordination partners to identify and prioritise people experiencing chronic homelessness who require in-depth assessments that current service system efforts have been unable to secure, and further assist the Project Implementation Group to establish a Service Coordination Group with the objective of ending rough sleeping homelessness in Stonnington.
