An exciting opportunity exists for enthusiastic and experienced Senior Social Worker to join the Wellbeing Support Service for Older Adults at BHN. Helping them to understand and address the issues they face by tailoring bespoke, individualised plans to meet their needs. You will utilise your extensive practice knowledge to improve the wellbeing of older adults and collaborate with an innovative and supportive team.

This program provides outreach counselling and wellbeing support to individuals living in residential aged care facilities (RACFs) in the Stonington, Glen Eira and Port Phillip Local Government Areas. There is the opportunity to supervise Social Work students and leadership opportunities relating to stakeholder relationship management with RACF managers.

About the role:

  • Perform evidence-based psychological and psychosocial interventions to support older adults with a recovery-oriented focus.
  • Integrate into relevant RACFs and provide discipline-specific support to staff and management around treatment and referrals.
  • Collaborate with RACF staff and other care team members to build capacity.
  • Conduct comprehensive holistic assessments to inform individualised strategies and interventions promoting recovery-based outcomes.
  • Maintain up-to-date knowledge of best practices for treating older adults within RACFs.
  • Work collaboratively with consumers, their families, and significant others, delivering family-sensitive models of service.
  • Provide ongoing assessment and monitoring of client and student wellness and safety, using mental state and risk assessment skills and safety/wellness plans.

About you:

  • Experience and demonstrated ability in providing evidence-based therapeutic interventions and a commitment to the Social Model of Health.
  • Knowledge of best practices and effective treatments for older adults in RACFs.
  • Ability to work autonomously, exercising sound professional judgment and seeking advice when appropriate.
  • Relevant leadership experience, high-level supervision skills, and the ability to facilitate a positive learning environment for students.
  • High levels of emotional intelligence and the ability to effectively engage with multidisciplinary teams, consumers, their carers/families, and other stakeholders.
  • Ability to create a person-centred, outcomes-driven culture in psychological service delivery within the aged care sector.

Requirements:

  • Tertiary qualification in Social Work.
  • Membership or eligibility for membership of AASW.
  • Social Work - Mental Health Accreditation preferred but not essential.
